Count me as one of those who didn't figure it out at first. (And actually, this is the second Supernatural fic where the POV OC has been a ghost and I didn't pick up on it. And here, even when I realized that part, it took me a while to get the rest of the twist.)

Even the parts that were confusing in the first read were telling in what they revealed. The first moment I was somewhat befuddled (apart from the telekinetic shockwave, which is just...standard Winchester weird, I guess) was when the handcuffs had suddenly been picked. I was all "Wait, did Sam..? But that's Dean's gig." Which should have clued me in right there.
